I take this as a piece about someone very special to you, who has stuck with you, loved you, while you were perhaps going through a very rough time. Reads like you might have made it fairly difficult for her to love you, but she still did. The good thing is that you realise this now, and will make damn sure you keep hold of her.
That's how I read this piece, it's an excellent piece and a beautiful tribute to your love!
